Wellness West

Wellness West is founded on the recognition that many factors impact a person’s health: economic opportunities, education, housing, transportation, access to food, access to care and mental health services. By working together, a diverse range of service providers can do a better job of helping people connect to the right supports at the right time and provide care for one's total health and well-being. 

Wellness West is building a transformational continuity of care model on the West Side of Chicago that allows hospitals, health systems, community mental health centers, community health centers and social service organizations to share information and coordinate services with the goal of closing gaps in the system.  

Central to this model is the Community Health Worker (CHW) role.  ACCESS is proud to employ a team of Wellness West CHWs who work alongside our patients to serve as a personal guide and advocate through the complex maze of resources that are available, but often difficult to navigate and access.  

Wellness West is funded by a grant from the Illinois Healthcare Transformation Collaborative, which is being managed by the Illinois Department of Health and Family Services.
