Preparing For Your Visit

• Check in up to 48 hours in advance of your appointment time through your myACCESSHEALTH account. 
• Arrive 15 minutes early to your appointment. 
• Bring any current insurance information (your insurance card and/or Medicaid or Medicare card).  

To be screened for benefits or to apply for our sliding fee scale program, the items listed below must be given to our staff at the time of your visit. Your payment amount will be determined and is due at the time of your visit. 

• Photo I.D. 
• Proof of address (two pieces of mail or one piece of mail and a photo I.D. with address and name included) 
• Proof of income (income tax forms, two pay stubs, SSI/SSA official documentation, two most recent pay stubs, letter from employer, or ACCESS housing/shelter or employment verification forms) 
• Other documents may be required, please contact your health center for further requirements 

• If you have received care outside of ACCESS, please bring a copy of your medical records. 
• If you were referred to ACCESS, bring the referral form to your first appointment. 

• Bring your medications in their original containers or bring a list of the medications you are currently taking, including the strength and dosage of each prescription. You can also ask for a list of your medications from your pharmacist. 

• Provide a list of any over-the-counter drugs you are taking, including vitamins and supplements. 
• Bring the address and phone number of your preferred pharmacy. 
• Bring a list of any known allergies. 

Bring up-to-date information about your family medical history. If your health care provider knows about the different diseases or illnesses you are more susceptible to, he or she can recommend lifestyle changes, look for signs and symptoms of those illnesses, and assess your current risk level of the disease. 

• Make a list of any questions you have ahead of time, with your most important questions at the top of the list. We want to make sure you get the answers you need. For additional support, here is our Adult Wellness Guide to help you prepare for your annual visit.  
• Find out what exams, vaccinations, or tests you should be getting at your age. Make sure all of your vaccinations are up-to-date or ask about which ones you may need. 
• Do not be afraid to talk about sensitive subjects. 

• Be honest with the information you share with your provider. 
• Tell your primary care provider when you get care from somewhere else, like a hospital stay or emergency room visit. 
• Write down any instructions your provider gives you to help you remember them later. 
• Ask the provider questions when you don’t understand the instructions. 
• Talk with your provider about any questions you may have about advance directives, which allow you to provide instructions in case you become unable to make medical decisions in the future.
